And the question is...if you look on the minimap it shows a grenade icon. I have a smoke grenade model there but I didn't actually throw a grenade...is that why there is an icon for it?

yup i believe so.. i remember seeing a nade icon on charlie and thats what it is :) btw nice lookin map
